Shamouti Island is an extra island located in Kanto. It can be accessed by talking to Sailor Sam in Vermilion City docks. Visiting the island is necessary for the Legendary Birds (quest).

Upon arriving on the island you'll notice that the buildings are spread all over the place and if you're spotted by a police officer you'll be sent straight to the pokemon center. That'll change after you talk to Melody at the Gym like building at the North Western part of the island, who'll initiate the Legendary Birds (quest) by sending you to find the 3 orbs from Ice Island, Fire Island, and Lightning Island. Inside the same building you'll also find Ash, Misty and pikachu, though nothing but a simple dialogue comes out of that interaction.

By trying to swim north of the island you are blocked by a message that says "The seas seem too rough to Surf. Perhaps a Pokemon could be of aid. That is resolved by bringing along a Pokemon with the ability Cloud Nine.
